New patients:
Please note, when you book an appointment, you will then be emailed a link to fill out initial intake forms. We ask that you have the forms to the practitioner at least 24 hours before the appointment so that she has time to review them properly, which ensures you will receive the best care possible. Please note that your forms are sent to you in a separate email after you book your appointment. If you ever need to edit the forms, simply log in to your patient portal, input your username and password, and look at the top left for a section titled “Forms”. -Please note that the insurances we DO accept are CDPHP, MVP, Cigna, Aetna, United Healthcare, NYSHIP (Empire Plan).
-We do NOT accept no fault, workers comp, Fidelis, Humana, BCBS or Highmark. However, we DO provide a superbill for you to submit to your insurance company for out of network reimbursement.
-For all other patients our out of pocket rate is $100 for the first visit, and $80 for all follow ups.
